Pre-Consultation Form
1.
Please complete the pre-consultation form before your initial appointment. This questionnaire is an opportunity for you to share any information you would like me to have prior to the consultation, including any intentions/goals, accommodations and history you may be more comfortable sharing in written form. By providing this information, it allows me to better understand your unique needs and ensure the support I offer can be tailored accordingly.

Initial Appointment
50-60 minutes
Initial appointments are a time for us to connect and get to know each other and begin unravelling your unique food and body story. Rather than telling you what to do, I will provide guidance as you explore the areas you’d like to focus on or experiment with.
Please note that initial assessments typically take 2-3 sessions for us to feel like we’ve got a solid landing pad. It is really normal for us to ‘bounce’ around a little bit and to not get through everything in the first session! Some topics we may cover include; what your daily life looks like, your mental and medical health history as well as your relationship with food and body and eating patterns. Questions are always optional. We’ll check in at the start of the appointment around any topics that feel tricky to talk about.
Standard reviews are 50 minutes with 10 minutes of admin time used to complete notes and communicate with multidisciplinary team members as appropriate.
It’s here where the real puzzle starts to come together. We will start to co-create a roadmap together of what we would like our work to look like; how you would like to feel and experience your relationship with food and body differently. You may find that some sessions feel easy, while others feel more challenging – this process is messy and complex, and that’s perfectly okay.

Nope, not at all! Anyone can see me, with or without a referral from a doctor. A Medicare plan just allows you to receive a rebate from the Government. You can also check with your private health insurance to see if your plan covers dietetics - everyone’s is different!

There are a few Medicare plans your GP can provide, however the two most common ones for a dietitian are a Chronic Disease Management Plan and an Eating Disorders Management Plan. If we begin working together, and I believe you may be eligible, I will advocate for you to contact your GP. Receiving a rebate can enhance the accessibility of sessions.
